Health Streams

Health streams are directions of continued care for people or groups with conditions or diseases, or at risk of developing one or more.

A health stream is a care plan in its most complete form, and incorporates community and flexibility.

Chronic illnesses are important targets of health stream development. Examples of health streams include:

In this site, human biology and disease are integrated. Causes, progression, and effects disease are determined by tissue architecture and fuction, which, in turn, is illustrated by how things can go wrong.

For each health concern, there is a unique continuum (a health stream) which moves throughout the health care system ideally seamlessly, in both directions.

For example looking at cancer, public health would involve vaccinations and pesticide bans, community health would be fresh produce available, primary health would be screening, and acute care would be oncology.
